A multicontributed, definitive postgraduate textbook combining applied science with recommended clinical practice. It is designed to serve both the trainee needing a general account of a particular technique, disease or treatment and the established specialist seeking guidance on some rare condition. It is now five years since the first edition of this book was published and the philosophy and overall balance of the book is little altered. Most chapters have been revised extensively and many expanded to accomodate more detailed and up-to-date accounts offering more assistance to readers seeking information on less common conditions. As a result, the number of volumes has increased from one to two, contributors from 94 to 125 and editors from three to four.
